The Washington State Immunization Information System (IIS) (PDF) is a lifetime registry that keeps track of immunization records for people of all ages. The system is a secure, web-based tool for healthcare providers and schools. The IIS connects people who receive, administer, record, and order vaccines in Washington.

Current users access the IIS through Secure Access Washington by adding the IIS as a service. For more information about this process see the FAQ here (PDF).
These sharing agreements are for organizations and institutions, not individual users. Check with your organization to see if they are already enrolled. You can also check the Participating Organizations list (PDF).
The Department of Health Office of Immunization has upgraded to a digital webform for IIS access.
If you have difficulty filling out the webform please see the Digital Information Sharing Agreement Reference Guide (PDF) or this short video for step-by-step instructions. If you have reviewed the guide and are still experiencing difficulties, please reach out to the data quality team at pchoiisa@doh.wa.gov for support.
Please note: Paper applications post marked after 11/01/2023 will not be retained due to volume received. If you have submitted an application via mail after 11/01/2023, please complete a new digital application.

DOH works closely with our IIS third-party technology vendor to address and fix any technical problems that arise within the registry. We do our best to monitor these issues closely and communicate temporary problems to our stakeholders and IIS users. To drive down program costs, WA participates in a consortium with other states that use the same system. As a result, some upgrades and project priorities are subject to approval from the entire consortium. For comments, questions or concerns about the forecasting, functionality, or the user experience of the WAIIS, please contact the IIS Help Desk at 1-800-325-5599 or WAIISHelpDesk@doh.wa.gov.
